Policies & Fees

Health Insurance:

  • Health Peak is an out of network provider.
  • Health Peak will provide an invoice with codes needed for submission to your insurance company for reimbursement.
  • Payments are expected at the time of each visit.
  • Payments to be made with Cash or Check only. Credit cards are not accepted. 
  • You will be charged for a missed or cancelled appointment with less than 24 hour notice unless in the event of emergency or inclement weather.
